The Chartered Family Health Center has provided quality health care services to residents of the northeast DC community since opening its doors in 1997. Important information about walk-ins and same day appointments

• Walk-In hours are from 9:00am-10:00am Monday through Friday. Check in for walk in patients begins at 8:15am.
• A walk in visit is a short, sick visit (10 - 15 min) to solve one urgent issue.
• Same day appointments are scheduled through calling the fast track nurse response line starting at 8:00am.
• The fast track line phone number(s) are 202-398 -8684 or 202-398-8683 and select option #2.
• All members need to arrive 30 minutes prior to their scheduled appointment(s).

• It is your responsibility to know your medication(s). Prescriptions are valid for 1 day to 3 months depending on the type of medication you were prescribed. Pay attention to how many refills you were given and how many remain after a trip to the pharmacy.
• Schedule your appointment(s) two weeks before your medicine is finished to avoid running out.
• You will need to be examined by the Doctor before more medicine is prescribed.
• All Chartered members seen outside of the center must follow up with their medical doctor, and have your medical records forwarded to your medical doctor. READ your discharge instructions and bring all discharge instructions with you for your follow up appointment.
• A referral is needed for services done outside of CHARTERED FAMILY HEALTH CENTER and needs your primary care doctor's approval first. Any referral requests must be submitted with supporting documents. Please allow 72 hours processing time. Referrals are mailed and usually take 3 to 5 business days.
• Physicals, PAP smears and PPD's (skin test) results are only good for 1 year.
• An RN must read PPD's before forms can be submitted to medical records to be completed.

Other helpful information

• It takes 7-10 business days to have all MEDICAL FORMS completed prior to pick up.
• CHILDREN are NOT ALLOWED in the OB/GYN EXAM AREA unless seated in a car seat or stroller.
• Members/Visitors are not allowed to sit/stand around the security desk.
• ABSOLUTELY NO FOOD OR DRINK ALLOWED IN THE CENTER.
• ABSOLUTELY NO CELL PHONE USE ALLOWED IN THE CENTER.
• Metro Asthma & Allergy Center hours are Monday 1pm-6pm, Thursday & Friday 9am-1pm and closed on Tuesday.
• Immunizations are for established patients only (not for patients visiting for the first time). Immunization walk-ins are for ages 3 through 17 ONLY. Hours of operation are Monday, Tuesday and Wednesday from 9:00am-11:00am and 2:00pm-3:00pm.
