Immunization Schedule

Chartered Health Plan encourages parents to make sure their child has regular check-ups and that all immunizations are up-to-date. Making sure that your child is healthy and properly immunized is a way to protect them from preventable and possibly life-threatening diseases. The 2008 Immunization Schedule is available for your reference. The schedule covers ages 0-6 (Childhood) and 7-18 (Adolescent). A Catch-up Schedule is also included. Changes in 2008 include a dose of pneumoccocal conjugate vaccine for healthy children ages 24-59 months who are completely immunized, increased age ranges for influenza vaccine, and preference for MCV4 over MPSV4 in high-risk children ages 2-10.

Click Childhood Immunization Schedule to view a copy of the Schedule for ages 0-6 years old.

Click Adolescent Immunization Schedule to view a copy of the Schedule for ages 7-18 years old.

If your child’s immunizations are not up-to-date, click Catch-up Schedule to view a copy of the Schedule to help you determine the minimum intervals between doses required to help your child catch-up. Make sure you use the schedule appropriate for your child's age. If you have questions, ask your child's Primary Care Providers.
