HealthChecks are regular check-ups for Chartered Health Plan members that start at birth and continue until you or your child turns 21 years old.  HealthChecks are FREE and help to ensure proper growth and good health.

HealthChecks include:

	A Complete Physical
	Immunizations (vaccines)
	Vision (Eye) Screening
	Hearing Screening
	Lead Screening (for all members under 6-years old)
	Dental Screening
	Health Education
