Health Education and Wellness Programs
Chartered Health Plan offers a variety of Health Education
Classes free to our Members. These Classes are available to
our members with or without a referral, however we find that
provider referrals stress the need for educational intervention
to our members and help to facilitate timely follow-up with
providers.

Choices and Changes - Risk Factor Reduction Program
This class is held two Saturdays a month for four
hours each session. The focus of this class is on the effects
of unhealthy lifestyle behaviors, such as eating patterns
leading to obesity, smoking and the lack of physical activity.
A review of the prevention and management of cholesterol and
hypertension will be provided.
Ages: 12 and UP. LUNCH PROVIDED

Lifestyle Choices and Changes
II - Weight Management/Nutrition Support
This 10 week program will focus on learning the advance information
required to successfully change your eating habits and daily
routine to achieve and maintain significant weight loss. The
course will cover how to read a food label, counting calories,
meal planning, low fat/low calorie cooking demonstrations,
grocery shopping tips, exercise demonstrations including resistance
training and tips to coping with “stress” eating.
In addition, every class will provide an opportunity to discuss
your personal barriers to weight management in a support group
format.

Parenting in the Real World - Kids Don’t Come with Instructions-Parenting
Education
This 7-week interactive parenting class, held on Wednesdays,
is designed to help parents of children birth through 3 years
cope with stress so that they can be better, more effective
caregivers. The sessions of Parenting in the Real World will
cover normal child development, disciplining children, being
exhausted, bonding and attachment, balancing work and school
with family, and caring for oneself. This class encourages
parents to talk about their experiences, share ideas, and
come up with remedies for reducing stress that work for them
and their families.

Prenatal Care
This 7-week program, held on Wednesdays, is designed to help
the expectant mother learn how to properly care for herself
and unborn child. The following information is covered: anatomy
and physiology of the reproductive system, fetal development,
physical and psychological changes of pregnancy, common discomforts
of pregnancy, cesarean section, breast feeding, parenting
techniques, postpartum techniques, contraceptive methods and
sexually transmitted diseases.

Project Move I
This is an aerobic exercise program, held on Tuesdays, in
partnership with Providence Hospital that provides a cardio
workout with strength resistance training.

Project Move II
This fun-filled class is held every Saturday and is a great
way to keep children and their families energized! It combines
fitness activities, weight management, nutrition, health education,
and dancing! It is family-centered so children, parents/guardians,
etc. are welcome to join.

Smoking Cessation
This class is offered the first Tuesday of each month in
partnership with Howard University to help smokers kick the
habit. While a number of individuals quit smoking on their
own, some need the help of others. This program helps the
member understand why they smoke and how to quit.

Sugar Busters - Comprehensive
Diabetes Education Program
This class is held two Saturdays a month for four hours each
session. The program provides education and training for individuals
with pre-diabetes (strong family history of diabetes with
elevated blood sugars) and/or diabetes. Participants are helped
in developing their own individual management plan.

Waiting to Exhale - Asthma
Management Program
This class is offered the second Monday of each month educating
individuals to recognize the warnings of asthma, and how to
control it and prevent attacks. This program empowers individuals
to make decisions about managing their asthma.
